Dear Mrs Smith,
Songs to Teach the Faith: Songs to Keep the Faith
A month ago I wrote to you about my research project, Songs to Teach the Faith: Songs to Keep the Faith, inviting your school to take part through the completion of a brief questionnaire (a copy of my original letter is enclosed).
I would still very much like to hear from you. I realise that your time is precious, and of course you are under no obligation whatsoever to help me, but in case my first letter never arrived, got lost, buried, or forgotten, I am taking the liberty of sending you another questionnaire and pre-paid return envelope.
The questionnaire has a return date of 14th October printed on the bottom. Please ignore this - it's not too late. If you are willing to take part, please complete the questionnaire and return it as soon as you are able.
If, for whatever reason, you do not wish to take part in the research, I would appreciate confirmation by the prompt return of the incomplete questionnaire.
